that Nigerian Nurses Charitable Association UK can treat
this and all donations I make from the date of this declaration,
until I notify you otherwise, as Gift Aid donations and
reclaim the tax.Notes:

Gift
Aid allows us to reclaim the tax you have paid on
all donations to Nigerian Nurses Charitable Association
UK’s work. This increases the value of your
donations by almost one third and will cost you
nothing. The income tax reclaimed will be used to
help fund the whole of Nigerian Nurses Charitable
Association UK’s work. |

Nigerian Nurses
Charitable Association UK can reclaim the tax on
your donation at the basic rate provided the amount
of income and /or capital gains tax you pay is equal
to the amount Nigerian Nurses Charitable Association
UK claims. |

If you pay a
higher tax rate you can claim further tax relief
in your Self Assesment Tax return. |

Completing the
Gift Aid Declaration does not affect your tax status
or commits you to supporting Nigerian Nurses Charitable
Association UK’s work for a specified length
of time. |

This declaration
covers all donations you make in the future. If
at a later date your circumstances change and you
no longer pay income and/or capital gains tax equal
to the amount Nigerian Nurses Charitable Association
UK are claiming you can cancel your declaration.
